Complex wounds are hard to catch early and easy to lose track of during care transitions. Left unaddressed, they can lead to infections, hospital stays, and real setbacks. Our clinicians look for the root cause, often vascular disease, not just the wound on the surface, and we stay connected with patients across care settings, so progress doesn't get lost after discharge. For us, partnership means helping facility teams prevent complications, avoid unnecessary hospitalizations, and deliver great care with confidence.
