Position Summary
Nurse Practitioner or Physician Assistant to provide high-quality care for long-term care residents by assessing, diagnosing, treating, and coordinating care alongside physicians and the interdisciplinary team.
Key Responsibilities
- Conduct adult/geriatric assessments and physical exams
- Manage acute and chronic health conditions
- Collaborate with physicians, families, and care teams
- Support care transitions and treatment planning
- Participate in quality improvement initiatives
Qualifications
- Active NP or PA license in the state of practice
- Board certification (AANP, ANCC, or equivalent)
- Minimum 2 years of clinical experience
- Active DEA license
- Current NPI and Medicare provider enrollment
- CPR certification
